Carers' Resource: Home From Hospital

Going home after a spell in hospital can be a daunting prospect for many patients.

After proving its success and securing investment from the local NHS, the Home from Hospital Service has been able to broaden the scope of the scheme to include adults of all ages living in Bradford, Airedale and Wharfedale. Following discharge, the Home from Hospital team helps patients with their immediate needs, such as making sure they have food and checking heating and lighting.

Additional support has included escorted shopping trips to build confidence, help with understanding medication routines, organising safe and sound alarms, benefits guidance and signposting to support groups.
